The information I usually collect
- Name
- Home address
- GP practice
- Email address (replaced with initials or code)
- Mobile number
- I don’t keep notes about our work; I rely upon my memory for key information
- If your health insurer will be paying for my fees, I will email them with your policy details/date of birth/name and address via my invoice. Each health insurer will have its own GDPR scheme
How do I use this information
- To contact you about appointment times/changes
- To invoice you (if arranged in advance)
- I will NOT use your information for any marketing reasons
- In the event of my believing you are at risk of harming yourself or others, I reserve the right to break confidentiality and contact your GP in the first instance
How long do I keep your information?
- Once our work has ended, I delete/shred all information I may hold (emails/texts/paper-held details)
- At any time before then, you have the right to be informed about the data I have and to receive a response within a calendar month, to withdraw consent to its use and to change/delete information
How do I protect your information?
- My smartphone and computers are password protected
- Your name is replaced with initials or code
- Any paper notes are kept in a drawer that is locked
